How It Works

1. Fill out our contact form here to request an appointment or via phone at 1-855-778-2511

2. If you are having a specialty kit collection, please fill out the enclosed patient requisition form prior to your appointment.

3. If you are having a standard or STAT blood draw, you MUST have a physician order. Please email or fax this information along with a copy of your photo and your insurance card to email: info@etamedicalservices.com or Fax: 1-877-943-8933

4. If you do not have a physician’s order, we can help with this.

5. Follow your doctor’s instructions to prepare for your blood draw, whether that is fasting or drinking plenty of water.

6. Relax! Our phlebotomist will perform the required blood draw in the comfort and convenience of your home or office and will then deliver the specimen to the designated lab for processing. We follow all required protocols, ensuring compliance.

Cancellation & Rescheduling

If needed, you can reschedule your appointment up to two times. Cancellations must be made at least twenty-four (24) hours prior to the scheduled time to avoid charges. Please note that no-show appointments are non-refundable.

Insurance coverage

We do accept all insurances for the cost of all laboratory test, however we are not currently accepting insurances to cover the cost of the collection fee which includes the collecting, processing and shipping/transporting to the laboratory.
